NIH Prior Approvals
This page provides a list of information to be included in prior
approval requests to the NIH. A FormsNirvana form is under construction.
Proposal Routing Form
The PRF (Application for External Research, Training, or Public
Service Support, formerly known as the BA23) is used to obtain
internal approvals necessary to submit an application for external
research or training support and to gather information for the
institutional proposal and award databases.
Faculty and staff prepare the PRF using the EGMS Web site (Electronic
Grants Management System; http://egms.umn.edu.
Material Transfer Agreement Routing Form (MTARF)
The MTARF is available as a Web-based form in EGMS (Electronic
Grants Management System; http://egms.umn.edu).
The EGMS MTARF requires the use of the EGMS electronic routing
feature for collecting approval signatures. For information about
EGMS electronic routing, call the EGMS business helpline at 612.624.1033.
As of January 1, 2003, the MTARF Word template is no longer available
at this site. After January 1, all new MTARF's must be prepared
using EGMS; SPA will no longer accept MTARF's prepared using the
Word template.
CUFS/Sponsor
Proposal Budget Worksheet
Form used to provide budget information to SPA for grant establishment
(commonly referred to as CUFS Friendly Budget).

A confidential document by which University faculty can describe
a new technology and ask OTC staff to evaluate its patenting and
